I like the initial wording of #1 with the choices from #2. Like this:

I like number one. It is simple but covers everything that the rsvp needs to cover I don't see the point in confusing things any further...
ETA: I like the combination choice that Lizbet just added too.

I would go with this, but I would say "Unable to attend"___

The only thing I can think might trip you/guests up is the "number attending" line. Since it is followed by the "number of children" attending line, are they putting the TOTAL number of people attending (and letting you subtract the children to figure out the adults attending) or are they putting the number of ADULTS attending?? And you may get mixed results, leaving you to figure which option they meant.

I might put something like this:

The favor of a reply is requested
on or before DATE
M ____________
Number of adults attending ____
Number of children under 12 attending _____
Unable to attend ____

ok I like what everyone is saying here. You are helping me out. But I can not do the under 12. The first group is 0-3 years then the next group is 4-6.

So how about this:

sounds good to me! The reason she needs the ages is because there is different prices for different ages.

I like that better where you just ask for the ages of kids attending because I would have been confused how many to put on the first line if you weren't specific and that could have been a big mess when you got them back with different people interpreting differently.

Yeah, but if the inivte was for a family of 5 but only 4 were attending it'd be quicker to see on the last line exactly how many couldn't make it.

If you didn't have that line there you would have to refer back to your guest list for every RSVP received just to check if everyone who was invited was going to attend.
