scratching chin Wording of RSVP

Hi,
Instead of having my guest mailing their rsvps I would like them to reply by email or phone...how do I word it?

You would print up a response card giving the instructions that might read,

The favour of a reply is requested on or before (date)
Please contact us at
(e-mail)
or
(phone)
with your full name and number of guests.
We look forward to hearing from you!

You can also put other details on this, such as # of child guests under age 10 -- since some caterers don't charge for kiddie meals -- and entree choice if you'll allow guests to select
